Certificate of Attendance

Ohio TESOL will provide a PDF certificate based on your attendance in any of our events.

Indicate the participant’s name, the event attended, and the certificate will be sent to the email address we have on file.

Logged-in members and student members will have a %50 discount on certificates.

In addition to paying this fee, please fill out the following form and provide some evidence to verify your attendance in Ohio TESOL events.

**You only need to pay once for each series as long as all documentation is submitted at the same time.**
